1. Data Privacy
- The Integrated Human Resource Management Information System (IHRMIS) operated by Cagayan Valley Medical Center ("CVMC") complies with the Data Privacy Act of 2012 of the Philippines (Republic Act No. 10173). CVMC is committed to protecting your personal information and maintaining its confidentiality. By activating your account and using IHRMIS, you consent to the processing and storage of your personal data in accordance with the Data Privacy Act of 2012.
2. Information Collected
- Types of Data: Personal Information (Name, Birthday, Sex, Address, Contact Number), Employment Details (Position, Department, Date of Employment, Nature of Appointment), Payroll Information, Leave Records, Performance Evaluation Data, Training and Seminars, and other HR-related records.
- Account Credentials: You are responsible for maintaining the confidentiality of your IHRMIS account credentials. Do not share your password with anyone.
- Accuracy: You confirm that all information you provide during registration and in subsequent transactions within IHRMIS is true, accurate, and complete. Intentional misrepresentation may lead to disciplinary action.
- Supporting Documents: Documents uploaded to IHRMIS (PDF format) must be authentic. CVMC reserves the right to verify any uploaded document.
3. Use of Information
- Your personal and employment data within IHRMIS is used solely for legitimate HR management purposes, including but not limited to: payroll processing, leave administration, performance evaluation, training management, and compliance with government requirements.
- Confidentiality: Your data will be treated confidentially and will not be shared with unauthorized parties without your explicit consent, except as required by law or government regulation.
- System Access: Access to IHRMIS is granted based on your role and designation. You agree to use the system only for its intended purpose and within your assigned access level.
4. User Responsibilities
- You are responsible for all activities conducted under your IHRMIS account.
- Report any unauthorized access or suspicious activity to the CVMC-HRMS Office immediately.
- Do not attempt to access systems, data, or features beyond your authorized access level.
5. Data Security
- CVMC employs reasonable technical and organizational measures to protect your personal information within IHRMIS. However, no system is completely secure.
6. Changes to Terms and Conditions
- CVMC may update these Terms and Conditions at any time. Changes will be communicated through official CVMC channels or posted within IHRMIS.
- Continued use of IHRMIS after any such changes constitutes your acceptance of the updated Terms and Conditions.
